Documents I Made
- Affidavit (pdf)- This is a one page affidavit. You can add multiple children’s names. This is the one I use.
- Unsworn Declaration(pdf) – While the home education law didn’t change, the State law allows for occasions when a notarized affidavit is required an unsworn declaration may be used (42 Pa. C.S. § 6204: Act 2020-15 of 2020). You may use this at your discretion. I did place wording the declaration is evidence as the law states the affidavit is evidence.
- homeschool-transcript (doc) You may edit this document to suit your needs. Grading system may differ from yours. If a school desires it, you may want to add on course descriptions in additional sheets. I have used this to send to my own child’s college and it was accepted.
- Exemption Letter(pdf)
- evaluation (pdf) – Your evaluator may have their own form. I want you to note that I do not include any area for your evaluator to supply “proof” of their eligibility to evaluate. This is because that’s not required by law. {this assumes your person does not fall into the “other” category} This means no “certificate number” or “PPID” number are required because it is not up to the school to determine who satisfies the law requirement for your evaluator. That burden is on you.
- graduate evaluation (pdf) – An evaluation letter for your graduating teen. (This version is not required. I developed this to ensure my home district understands my child is completed.)
- Simple Report Card – (pdf) I created this very simple report card. Use it as you wish.
- Editable Report Card (doc) Feel free to edit this for your personal use. I have needed a “report card” for my children’s employment and for obtaining bank accounts. I tried to explain they were home educated, but that goes over people’s heads. So, this was sufficient for them.
- attendance record (pdf) Very easy way to keep track of your days. This is NOT required by law to be in your portfolio but this can help keep you on track. Remember, you attest to 180 days (or required hours) in your affidavit which is a legal document.
- materials log (doc) – You need to keep the log updated as you go through the school year. List the title of each book used for instruction. The law says, “a log, made contemporaneously with the instruction, which designates by title the reading materials used..”. You do not need a daily book log or to record every book each day. If you wish, you can add the date you began and/or the date you finished the book but this is not required. Also, the log in the law is NOT an attendance log! It does not matter what your evaluator tells you.

These are items I did not create.
- PA affidavit – (pdf) This is a one page affidavit for one child’s name and age. I have used this one also but added additional space for my 2nd child.
- elementary objectives – (pdf) Simple and to the point
- secondary objectives – (pdf) objectives don’t need to be elaborate or contain your curriculum choices
- “Official” immunizations card – (pdf) This is from the PA Dept of Health
- medical examination form – (pdf) If you’re looking for the old PA Dept of Health medical exam school form (they’ve adopted a new more intrusive one in recent years), this is a copy of that form. (Note – The PA Homeschool Law does not explicitly require you turn these forms in to your district or that you must have them filled out at all.)
- private dentist report (Pdf) – This is a copy of the private dentist form if you choose to have it filled out and keep for your own records, not required to be submitted to school.
- PDE Diploma – providing the link to the Pennsylvania Department of Education printable diploma for any interested persons.
